45–60 Minute Evaluation
A comprehensive assessment allows us to understand your condition, movement patterns, and goals.
Personalized Home Exercise Program
We'll provide exercises and education to help you continue making progress between visits.
One-on-One Care
You'll work directly with your physical therapist in a supportive, individualized environment.
Insurance Verification
Our team will verify your benefits and help answer questions about coverage before treatment begins.
What to Bring
Help us make your first visit as smooth as possible by bringing:
Photo ID
Insurance card(s)
Physician referral (if required)
Any imaging reports or surgical notes related to your condition
A list of current medications
What Should I Wear?
Wear comfortable clothing that allows you to move easily.
Depending on the area being treated, you may want to wear:
Athletic clothing
Shorts for knee, hip, or ankle injuries
A tank top or loose-fitting shirt for shoulder or neck treatment
Comfortable athletic shoes
The more easily we can evaluate movement, the better.
